‘உங்களுக்கும் சுகர் இருக்கா?’ என்று சாதாரணமாக விசாரித்துக் கொள்ளும் அளவுக்கு இன்று சர்க்கரை நோயும் பரவலாகிவிட்டது. அதன் சிகிச்சைகளும் எளிதாகிவிட்டன. காரணம், நீரிழிவுக்கான ஆணிவேரையும் தீர்வையும் நாம் கண்டுபிடித்துவிட்டோம்.

நாம் உண்ணும் உணவு குளுக்கோஸ் எனும் சர்க்கரையாக ரத்தத்தில் சென்று மாற்றமடைகிறது என்பதையும், அந்தச் சர்க்கரையின் அளவை கணையத்தில் சுரக்கும் இன்சுலின் கட்டுப்படுத்துகிறது என்பதையும் 1869ம் ஆண்டு ஜெர்மனை சேர்ந்த மருத்துவக்கல்லூரி மாணவர் பால் லாங்கர்ஹேன்ஸ் என்பவர்தான் முதலில் கண்டுபிடித்தார். சாதாரணமாக இறந்தவர்களின் உடலையும் நீரிழிவு பாதிப்பால் உயிரிழந்தவர்களின் உடலையும் ஆராய்ச்சி செய்தபோது நீரிழிவாளர்களின் கணையத்தில் இருந்து ஏதோ ஒரு திரவம் குறைவாக சுரப்பதைக் கண்டுபிடித்தார்.

பால் லாங்கர்ஹேன்ஸின் கருத்தை உறுதிப்படுத்துவதற்காக, 1889ல் ஜெர்மனியை சேர்ந்த ஆஸ்கர் மின்கோவ்ஸ்கி மற்றும் ஜோசப் வான்மெரிங் ஆகிய இரு மருத்துவர்கள் ஒரு நாய்க்கு கணையத்தை அகற்றிவிட்டு, நடப்பதை வேடிக்கை பார்த்தார்கள். கணையத்தை இழந்த பின் ரத்தத்தில் சர்க்கரை அளவு அதிகரித்து, நாய் இறந்துபோனது. கணையத்தில் சுரக்கும் ஏதோ ஒரு வேதிப்பொருள்தான் இதற்குக் காரணம் என்று தெரிந்தது. 1910ம் ஆண்டு சர் எட்வர்ட் ஆல்பர்ட் ஷார்பே என்பவர், இந்த வேதிப்பொருளுக்கு இன்சுலின் என்று பெயர் வைத்தார். லத்தீன் வார்த்தையான Insula என்பதில் இருந்து Insulin என்ற வார்த்தையை உருவாக்கியவர் இவர்.

1921ம் ஆண்டு டொரோண்டோ பல்கலைக்கழகத்தில் ஃப்ரெடரிக் பேண்டிங், சக மருத்துவர் சார்லஸ் பெஸ்ட் இருவரும் 10 நாய்களின் கணையத்தை அகற்றிப் பாதுகாத்து வைத்து விட்டு, அவற்றுக்கு சர்க்கரை அளவு அதிகமாகும் என்பது தெரிந்தே காத்திருந்தார்கள். சர்க்கரை அளவு அதிகமானவுடன் அகற்றப்பட்ட கணையத்தில் இருந்து இன்சுலினை எடுத்து, நாய்களுக்கு ஊசியின் வழியாக செலுத்தினார்கள். இன்சுலின் செலுத்தியவுடன் நாய்கள் ஆரோக்கியமாக இருந்தன.

ஆகவே, இன்சுலின் சுரப்பு குறைந்தால் ஊசியாகப் போட்டுக்கொண்டு சமாளித்துக் கொள்ளலாம் என்ற முடிவுக்கு வந்தார்கள். அடுத்து முயல் குட்டி, பசு, பன்றி என விலங்குகளின் கணையத்தில் இருந்து இன்சுலினை தயாரிக்க ஆரம்பித்தனர். 1922ல், டொரோண்டோ மருத்துவமனையில் நீரிழிவுக்காக சிகிச்சைக்கு வந்த லியானார்ட் தாம்ஸன் என்ற டீன் ஏஜ் பையன்தான் இன்சுலின் ஊசி பயன்படுத்திய முதல் பயனாளர்! நாளடைவில் மிருக வதைக்கு எதிரான குரல்கள், அறிவியலின் வளர்ச்சி, இன்சுலினின் பிரமாண்ட தேவை போன்ற காரணங்களால் பயோடெக்னாலஜி, வேதியியல் தொழில்நுட்பம் ஆகியவற்றின் மூலம் ஆய்வகத்திலேயே இன்சுலின் தயாரிக்கும் முறை இப்போது உருவாகிவிட்டது!

ச ர்க்கரை வியாதிக்காரர்கள் நாய், ஆடு, மாடு, பன்றிக்கெல்லாம் மிகவும் கடமைப்பட்டவர்கள் என்று சொன்னால் அது பெரும் உண்மை. அதுவும் குறிப்பாக நாய்க்கு ரொம்பவுமே நன்றியுடையவராய் இருந்தே ஆக வேண்டும்.மூன்று அல்லது நான்காயிரம் வருடங்களாக உருக்கி உருக்கி உருக்குலைத்துக் கொல்லும் நோயாக, ‘முகம் தெரியா பிசாசு’ என்றழைக்கப்பட்ட நீரிழிவு நோயைக் கட்டுப்படுத்த 20-ம் நூற்றாண்டின் விடிவெள்ளிக் கண்டுபிடிப்பாக வந்ததுதான் ‘இன்சுலின்’. நீரிழிவைக் குணப்படுத்தும் மருந்தல்ல இன்சுலின். ஆனால், மிக நுட்பமாகக் கட்டுப்படுத்தும் மருந்து.தொடையில் பந்தைத் தேய்த்து, பாகிஸ்தான் கிரிக்கெட் வீரர் வாசிம் அக்ரம் 140 கி.மீ. வேகத்தில் பந்தை வீசவும், எல்லைகள் தெரியாத கடலில் சின்னதாய் ஒரு கப்பலில் இருந்துகொண்டு ஆண்டன் பாலசிங்கம் தமிழ் ஈழப் போராட்டத்தில் சுமார் 40 அல்லது 50 ஆண்டுகளுக்கு அரசியல் வியூகம் அமைத்து நடத்தியதும் இந்தச் சின்ன ஊசியைப் போட்டுக் கொண்டுதான்!

நாய் தந்த நன்மை

அதெல்லாம் சரி, இதற்கும் நாய்க்கும் என்ன சம்பந்தம்? நாய்களை வைத்துத்தான் ஒட்டுமொத்த இன்சுலின் ஆய்வும் நடந்திருக்கிறது. நாய்களின் கணையத்தைக் கழற்றிப் போட, தடாரென நாய் மெலிந்துபோய் சுகர் பேஷண்ட் ஆனதை ஜெர்மனியில் பல மருத்துவ ஆய்வாளர்கள் கண்டறிந்திருந்தனர்.
ஆனால், கனடாவைச் சேர்ந்த பேண்டிங் மட்டுமே, ‘அதன் நாளத்தின் வழியாக அல்லாமல், குறிப்பிட்ட பகுதியில் சுரக்கும் பழுப்பான திரவப் பொருள்தான், இந்த இனிப்பு சமாச்சாரத்தைக் கவனமாகக் கையாளுகிறது’ எனும் தகவலைக் கண்டறிந்தார். பேண்டிங் பெரிய மருத்துவப் பட்டம் பெற்ற இளைஞர் கிடையாது. என்றாலும், இந்த நோய்க்கு மருந்து கண்டறியும் ஆர்வம் மட்டுமே அவரிடம் இருந்தது.

ஆய்வுக்காக, கனடாவில் இருந்து கப்பலேறி இங்கிலாந்துக்குப் போய், அங்கே மிகப் பெரிய உடல் இயங்கியல் (physiology) விஞ்ஞானியாக இருந்த பேராசிரியர் மெக்லியாய்டுவின் ஆய்வகத்தில் தன் ஆய்வுச் சிந்தனைக்கு உதவி கேட்டார். பெரும் புறக்கணிப்புக்குப் பின்னர், ‘அட்லீஸ்ட் எல்லோரும் கோடை விடுமுறைக்குப் போகும்போது, எனக்கு ஆய்வகத்தையும் இரண்டு உதவியாளர்களையும் மட்டுமாவது கொடுங்கள்’ எனக் கேட்டுப்பெற்றுத்தான் பேண்டிங் இந்த ஆராய்ச்சியைத் தொடர்ந்திருக்கிறார்.

பகிர்ந்துகொண்ட மருத்துவர்

மாதத்துக்கு 100 டாலர் சம்பளம், கூடவே பெஸ்ட் எனும் ஆராய்ச்சி மாணவருடன் இந்த ஆராய்ச்சி தொடர்ந்தது. ஆராய்ச்சிக்குப் போதுமான நாய்கள் கிடைக்கவில்லை. இரவு நேரத்தில், தெரு நாய்களை 1 டாலருக்கும் 2 டாலருக்கும் சொந்தப் பணத்தில் பிடித்து வந்து, இரவும் பகலும் ஆய்வு நடத்தியிருக்கிறார்கள். தொடர்ச்சியான ஆய்வின் முடிவில், 1922-ல் நாயின் கணையத்திலிருக்கும் சாரத்தை (extract) அவர்கள் பிரித்தெடுத்தார்கள். அதற்கு ‘இன்சுலின்’ எனப் பெயரிட்டனர். பிறகு கோலித் எனும் இன்னோர் அறிஞருடன் சேர்ந்து பல ரத்த மாதிரி ஆய்வுகளை நாய்களில் மேற்கொண்டார்கள். கடைசியாக, கனடா நாட்டு டொரண்டோ நகரத்தில் மரணத்தின் விளிம்பில் இருந்த இளைஞன் ஒருவனின் ரத்தத்தில், அந்தச் சாரத்தை நேரடியாகக் கலந்தபோதுதான், சர்க்கரையைக் கட்டுப்படுத்தும் இன்சுலின் உலகுக்குக் கிடைத்தது. இந்தக் கண்டுபிடிப்புக்காக பேண்டிங், மெக்லியாய்டு இருவருக்கும் நோபல் பரிசு கிடைத்தது. அந்த ஆராய்ச்சியில் துணை நின்ற கோலித், பெஸ்ட் உடன் பரிசையும் பரிசுப் பணத்தையும் அவர்கள் பகிர்ந்துகொண்டார்கள். பென்சிலின்போல், போலியோ தடுப்பூசிபோல் உலகின் மிகச் சிறந்த மருத்துவக் கண்டுபிடிப்பு இன்சுலின். இன்றைக்கு இன்சுலின் என்பது ‘பில்லியன் டாலர்’ லாபம் புழங்கும் வணிகமாக மாறிவிட்டது!

குணப்படுத்தாது… கட்டுப்படுத்தும்!
‘அய்யோ இன்சுலினா?’ எனப் பயப்படுகிற சாமானியர்கள் ஒருபக்கம். ‘இன்சுலினை நேரடியாக அளிக்கும் அணுகுமுறை, முற்றிலும் தவறு’ எனச் சாடும் மருத்துவர்கள் இன்னொரு பக்கம் என்ற நிலை இன்றும் தொடரத்தான் செய்கிறது. ஒரு விஷயத்தைத் தெளிவாகப் புரிந்துகொள்ள வேண்டும். இன்சுலின் நீரிழிவு நோயைக் குணப்படுத்தும் மருந்தல்ல. அதைக் கட்டுப்பாட்டில் வைக்கும் என்சைம். கணையத்தில் உள்ள செல்கள்தாம், என்சைமைச் சுரக்க வைக்க வேண்டும். நோய் எதிர்ப்பாற்றல் சரியாக இல்லாமல் போனால், அந்த என்சைம் சுரப்பது நின்று போகும். அந்த நிலையில், இன்றளவில் எந்த மருந்தையும் கொடுத்து, அந்த என்சைமை அதிக அளவில் சுரக்க வைக்க முடியாது என்பதே நிதர்சனம்.

அதனால்தான், இளமையில் வரும் குழந்தை நீரிழிவு நோய் (Juvenile diabetes), இன்சுலின் சார்பு நீரிழிவு (Insulin Dependent Diabetes-IDDM) நிலைகளில் இன்சுலினைத் தவிர்த்துவிட்டு, மரபு மருத்துவ முறைகளை, மூலிகை மருந்துகளை, சித்த மருத்துவத்தை மட்டும் நம்பியிருக்க முடியவில்லை. அதே இன்சுலின் சுரப்பு இயல்பாக உள்ளது. ஆனால், அதன் செயல்படும் திறனில் மட்டுமே பிழை என்றால் (Non Insulin Diabetes Mellitus -NIDDM), அத்தனை மரபு மருந்துவ முறைகளிலும் சிகிச்சையை மேற்கொள்ளலாம். சித்த மருத்துவ அனுபவம் இதில் பல வியக்கத்தக்க விஷயங்களை, கூடவே அதன் எல்லைகளை நிறையவே அடையாளம் காட்டுகிறது. ஆவாரம்பூவில் ஆரம்பித்து அழுத்தும் வர்மம், ஆயுர்வேதம், அக்குபிரஷர் சிகிச்சை, ஆசன சிகிச்சைவரை என்னவெல்லாம் நடக்கிறது? தொடர்ந்து பார்க்கலாம்!




Before the life-saving treatment of insulin was discovered, diabetes was a disease that many people feared they would get.  They feared it, because the diagnosis of diabetes was fatal, it meant they would most certainly die soon, in the near future.

Not much was known of diabetes before the discovery. Doctors knew that sugar would only worsen diabetic patient’s conditions, and the most effective treatment at the time was a limited diet, also known as a starvation diet. By reducing their sugar intakes to the bare minimum, patients were able to extend their lives by a few extra years. Sadly, these extra years were not full of life and happiness for them. They slowly became worse, and often times they died of starvation before they succumbed to the effects of the disease itself.

History of Diabetes Research
In the 19th century, when patients died from complications from diabetes, doctors were able to conduct an autopsy which showed significant damage to their pancreas. In the year 1869, Paul Langerhans, at the time a German medical student discovered that inside the pancreatic tissue were clusters of cells in which digestive juices were produced. The function of these cells were unknown during this discovery. Eventually however, some of these cells were found to be the beta-cells, which produce insulin. To honor the student that made this huge discovery, these cell clusters were called the “Islets of Langerhans”.

While this discovery in 1869 was huge in diabetes research, it still took some time to understand the functioning of the pancreas and its role in diabetes. In 1889, Oskar Minkowski, a German physiologist along with Doctor Joseph von Mering, were able to prove medically that if you removed the pancreas from a dog, the dog then got diabetes. However, if they surgically tied off the duct in which the pancreatic juices flowed, the dog only developed some minor digestive issues, but not diabetes. This research leads them to learning that the pancreas had two functions:

Production of a substance which regulates the blood glucose within the body
This research was a turning point in diabetes history. This meant that internal secretion of this substance was the key to unlocking the mystery that surrounded diabetes. Progression of this research and discovery of the mystery substance however took some time.

It wasn’t until October of 1920 that some significant headway was made in the discovery of insulin. Dr. Fredrick Banting, a surgeon in Toronto Canada, had an idea that the previously discovered pancreatic juices could be of harm to the secretion within the islets of Langerhans in the pancreas. What Banting wanted to do was to ligate the ducts in the pancreas to stop the flow of any nourishment that the pancreas receives. This would cause it to then lose its ability to produce any digestive juices. The cells which secreted the mysterious substance could then be taken from the pancreas.

In 1921, Dr. Banting presented his idea to the Professor of the University of Toronto, John Macleod. At the time Professor Macleod was the leading figure in the research of diabetes. Dr. Banting was able to convince him that this idea he had was worth a shot. With minimal equipment and 10 dogs for research purposes, the experiment that lead to the discovery of insulin had been started. Dr. Banting, along with the help of a research student named Charles Best, started their research and the outcome was as follows.

First Phase of Research and Testing
Dr. Banting and Charles Best, removed the pancreas from the dog. This resulted in:

Blood glucose rising
The dog became extremely thirsty and had to urinate more often
The dog became weaker.
The dog developed diabetes
Their next experiment on another dog included the ligation of the pancreas to stop the flow of juices and nourishment so that the pancreas would then degenerate. They removed the pancreas of the second dog, froze pieces of it in a mixture of salt and water. Once these were frozen they ground them up and filtered them out to make a substance known as isletin. The isletin was then injected into the first dog with diabetes. They now noticed and reported that its blood glucose levels had dropped and the dog became stronger. By giving the dog a few of these injections each day they were able to keep the dog free of any symptoms and healthy.

Dr. Banting and Charles Best reported their findings back to Professor Macleod who was definitely impressed but wanted more research and findings to prove how this extract really did work.

Second Phase of Research and Testing
To keep up with the demand of the extended testing, Dr. Banting realized he needed a much larger supply of pancreas, more than their dogs could provide. This is when they started to use cattle for their research subjects instead. They were able to manage to produce enough of their substance to keep many diabetic dogs alive and healthy.

These new results they uncovered convinced the professor that they were really on to something now. He was able to give them more money and a bigger laboratory to continue their research. Additionally, at this time he suggested that they call the extract, “insulin”.

While things initially started off slowly in diabetes research, once Dr. Banting and Best uncovered these discoveries, things really started to speed up.

In 1921, a new team member came on board, he was a biochemist named Bertram Collip. Collip was in charge of purifying the insulin so that they would be able to test it on humans.

Human Trials: The Real Test

Initially, the team was unsure of who to test their newly discovered substance on, so they started with themselves. They felt weaker and dizzy but really didn’t notice a difference and thankfully were not harmed. Collip worked on purifying the insulin better and also experimented so that he could figure out how to find the right dosage. He also was able to learn how to minimize the effects of an overdose with glucose. It was them when he found that orange juice and honey were great examples of glucose.

In January of 1922, Leonard Thompson was chosen as the first human with diabetes to receive the newly discovered substance of insulin. Leonard was only 14 and near death before the injections of insulin. The testing was a success, Leonard was able to regain his strength and also put on weight.

The team was so excited by the news that they were now able to extend their human trials and give their insulin to other patients with diabetes who volunteered to be treated. These patients also reacted just as positively as Leonard did.


Insulin Legacy

Dr. Banting, Professor Macleod, and the entire team patented their substance of insulin but ultimately decided to give away the rights to the University of Toronto for $1. This allowed the University to outsource the insulin formula to a medical firm by the name of Eli Lilly who was able to start a large production of the insulin. By 1923, they were able to produce enough of it to create a supply large enough for North America. The funds the University received from the income of insulin was able to later fund additional research in diabetes treatments.
While insulin isn’t a cure for diabetes, it was a lifesaving discovery that still saves the lives of many people living with diabetes today. Before it came along, diabetes was a death sentence, but now patients are able to live a full, normal, healthy life.

Insulin was discovered by Sir Frederick G Banting (pictured), Charles H Best and JJR Macleod at the University of Toronto in 1921 and it was subsequently purified by James B Collip.

Before 1921, it was exceptional for people with Type 1 diabetes to live more than a year or two. One of the twentieth century’s greatest medical discoveries, it remains the only effective treatment for people with Type 1 diabetes today.

First successful use
On 11 January 1922, Leonard Thompson, a 14-year-old boy with diabetes, who lay dying at the Toronto General Hospital, was given the first injection of insulin. However, the extract was so impure that Thompson suffered a severe allergic reaction, and further injections were cancelled.

Over the next 12 days, James Collip worked day and night to improve the ox-pancreas extract, and a second dose was injected on the 23 January. This was completely successful, not only in having no obvious side-effects, but in completely eliminating the glycosuria sign of diabetes.

A dramatic moment
Children dying from diabetic ketoacidosis (DKA) were kept in large wards, often with 50 or more patients in a ward, mostly comatose. Grieving family members were often in attendance, awaiting the (until then, inevitable) death. 

In one of medicine's more dramatic moments Banting, Best, and Collip went from bed to bed, injecting an entire ward with the new purified extract. Before they had reached the last dying child, the first few were awakening from their coma, to the joyous exclamations of their families.

The tale of the founding of Rome is recounted in traditional stories handed down by the ancient Romans themselves as the earliest history of their city in terms of legend and myth. The most familiar of these myths, and perhaps the most famous of all Roman myths, is the story of Romulus and Remus, twins who were suckled by a she-wolf as infants in the 8th century BC.[1] Another account, set earlier in time, claims that the Roman people are descended from Trojan War hero Aeneas, who escaped to Italy after the war, and whose son, Iulus, was the ancestor of the family of Julius Caesar.[2] The archaeological evidence of human occupation of the area of modern-day Rome, Italy dates from about 14,000 years ago.

The national epic of mythical Rome, the Aeneid of Virgil, tells the story of how Trojan prince Aeneas came to Italy. The Aeneid was written under Augustus, who claimed ancestry through Julius Caesar and his mother Venus. According to the Aeneid, the survivors from the fallen city of Troy banded together under Aeneas and underwent a series of adventures around the Mediterranean Sea, including a stop at newly founded Carthage under the rule of Queen Dido, eventually reaching the Italian coast. The Trojans were thought to have landed in an area between modern Anzio and Fiumicino,
southwest of Rome, probably at Laurentum or, in other versions, at Lavinium, a place named for Lavinia, the daughter of King Latinus whom Aeneas married. This started a series of armed conflicts with Turnus over the marriage of Lavinia.[3] Before the arrival of Aeneas, Turnus was betrothed to Lavinia, who then married Aeneas, starting the war.[3] Aeneas won the war and killed Turnus.[3] The Trojans won the right to stay and to assimilate with the local peoples. The young son of Aeneas, Ascanius, also known as Iulus, went on to found Alba Longa and the line of Alban kings who filled the chronological gap between the Trojan saga and the traditional founding of Rome in the 8th century BC.

Toward the end of this line, King Procas was the father of Numitor and Amulius. At Procas' death, Numitor became king of Alba Longa, but Amulius captured him and sent him to prison; he also forced Numitor's daughter Rhea Silvia to become a virgin priestess among the Vestals.[1]

The myth of Aeneas was of Greek origin and had to be reconciled with the Italian myth of Romulus and Remus. They were purported to be sons of Rhea Silvia and either Mars, the god of war, or the demi-god hero Hercules. They were abandoned at birth, in the manner of many mythological heroes, because of a prophecy that they would overthrow their great-uncle Amulius, who had overthrown Silvia's father Numitor. The twins were abandoned on the river Tiber by servants who took pity on the infants, despite their orders. The twins were nurtured by a she-wolf until a shepherd named Faustulus found the boys and took them as his sons. Faustulus and his wife Acca Larentia raised the children. When Remus and Romulus became adults, they killed Amulius and restored Numitor. They decided to establish a city; however, they quarreled, as Romulus was on Palatine Hill, while Remus wanted to found the city on Aventine Hill, until Remus and his followers attacked, and Romulus killed his brother.[1] Thus, Rome began with a fratricide, a story that was later taken to represent the city's history of internecine political strife and bloodshed.

The most familiar date given for the foundation of Rome, 753 BC, was derived by the Roman antiquarian Titus Pomponius Atticus, and adopted by Marcus Terentius Varro, having become part of what has come to be known as the Varronian chronology.[15] An anecdote in Plutarch where the astrologer Lucius Tarrutius of Firmum provides an argument based on a non-existent eclipse and other erroneous astronomical details that Rome was founded in 753 BC suggests that this had become the most commonly accepted date.[16] Through its use by the third-century writer Censorinus, whose De Die Natali was the ultimate influence of Joseph Justus Scaliger's work to establish a scientific basis of ancient chronology, it became familiar.[16]

Recent discoveries by Andrea Carandini on Rome's Palatine Hill have also yielded evidence of a series of fortification walls on the north slope that can be dated to the middle of the 8th century BC.[citation needed] According to the legend, Romulus plowed a furrow (sulcus) around the hill in order to mark the boundary of his new city
